  8. If you are registering as an individual, we will put you in a foursome of similar handicap golfers. If you are any other level sponsor, complete the information below on the other players in your foursome/s.
  9. If you are a Platinum, Gold, Sliver or Bronze sponsor, provide the names and handicaps for the additional players in your first foursome.
  11. If you are a Platinum or Gold sponsor, provide the names and handicaps of the players in your second foursome.
  13. If you are a Platinum Sponsor, please provide the names and handicaps for each golfer in your third foursome.
Other ways to participate and support the Benefit.
  1. If you are unable to attend the golf benefit during the day, but would like to attend the evening dinner, awards ceremony, and auction ONLY, please provide name/s of Dinner only attendee/s below.

    Dinner Buffet: $60.00 per person
    Reception: 4:00 PM with dinner to follow

  3. If your company does not wish to become a sponsor; however, you or your company would like to make a financial contribution to the Horace Mann School for the Deaf and Hard of Hearing wi will include your company or individual name as a Special Contributor in the program.
  5. If your company would like to contribute an item for the raffle, please provide contact information below, and somone from the Chapter will be in touch with you.
